Please follow this link to our secure web-page to pre-qualify your insurance and benefits information with our Billing Manager, Victoria House, at House Medical Billing Service.  Once you submit this form your information will be verified and you will receive a confirmation email with benefit details from Victoria.

Please submit this form prior to scheduling your first appointment. This form also includes credit card information for any client preferring to pay deductible, co-pays or other out-of-pocket expenses per your financial agreement. By supplying this information, you can receive eBills and eReceipts right to your email address and manage your payment options at www.MyProviderLink.com once your account is established.
